CRISP Event Notification Delivery (CEND)

The CRISP Event Notification Delivery (CEND) Solution is an internally operated system designed to offer real-time alerts for patients’ hospital encounters seamlessly through the Population Explorer application. CEND ensures timely troubleshooting and ongoing enhancements due to its internal nature, enhancing our ability to deliver accurate notifications. After submitting a CEND panel, users are able to view notifications for their patients through Population Explorer.

User Stories

In FY25, CRISP-DC aimed to understand how users of population explorer leverage the tool in their daily workflows and outcomes as a result of their utilization. Engagement occurred in one-on-one meetings where our team took time to discuss key features of population explorer, how data was shared across the care team, and opportunities users thought would improve their experience. These user stories can help organizations better understand how others approach utilization of Population Explorer and how it could be implemented within their organization. 

  • As a registered nurse for a Federally Qualified Health Center, I use Population Explorer to manage Non-Emergent ED utilization to help patients understand when it’s essential to go to the hospital, provide education and assistance in connecting patients with MCO-24-hour nurse advice lines
  • As a QI Data Analyst for a Behavioral Health Organization, I use Population Explorer to notify managers, and team leads after patient discharge. Our team educates patients about alternative options, such as urgent care, and helps them connect with primary care.
  • As a Business Change Manager for a Managed Care Organization, I use Population Explorer to manage nonemergent encounters and provide education on different care options to help lower ER Utilization and care costs
  • As an SUD/ACT Clinical Manager, I use Population Explorer to quickly identify consumers who need immediate mental health intervention, proactively support consumers with complex needs like homelessness, substance use issues, and medication non-compliance.
  • As the Chief Clinical Officer for my organization, I use Population Explorer to facilitate integrated care team delivery of transitions of care. Every Morning I use population explorer to export the last 72 hours of outpatient, inpatient and emergency encounters. I sort, highlight and assign encounters by team (integrated care, case managers, and therapy) to indicate priorities. This helps the integrated care team, case managers, and therapy teams know where to focus their efforts for the day. The notification service has had a significant decrease in hospitalization, and we have seen this since the COVID pandemic.
