CRISP DC is helping advance the District’s Transforming Maternal Health (TMaH) Initiative by supporting maternal health providers in their participation in health information exchange. Led by the DC Department of Health Care Finance (DHCF), the TMaH Initiative is focused on improving maternal health outcomes through enhanced care coordination, stronger provider collaboration, and better access to critical health information.

Through eHealth DC’s EHR technical assistance and onboarding services, CRISP DC is working to connect maternal health providers across the District to the DC HIE. As part of this effort, targeted technical assistance is being provided to 11 doulas and 5 physician groups/FQHCs to support their participation in maternal health data exchange and strengthen their ability to share and access critical patient information.

By expanding participation in the DC HIE, the initiative is helping bridge gaps between clinical and community-based care. Doulas, physician practices, and Federally Qualified Health Centers play an important role in supporting mothers throughout pregnancy and the postpartum period. Enabling these providers to participate in health information exchange helps ensure that care teams have access to more complete information and can better coordinate services across care settings.

Improved information sharing supports more informed decision-making, reduces communication barriers, and helps create a more connected care experience for patients. As maternal health providers become more integrated into the health information exchange ecosystem, they are better equipped to collaborate, coordinate care, and support the unique needs of mothers and families.

This work reflects CRISP DC’s ongoing commitment to leveraging health information exchange to improve health outcomes across the District. Through the TMaH Initiative, CRISP DC is helping build the infrastructure needed to support seamless maternal health data exchange, strengthen care coordination, and advance healthier outcomes for mothers and families.
