Are you looking for smarter, easier ways to ensure your patients get the preventive care they need? We’re excited to share that the CRISP DC Health Information Exchange (HIE) is rolling out powerful new tools designed to significantly improve your outreach efforts and help you close critical gaps in care.
Our focus is on making vital information accessible so you can target your efforts where they matter most. We’re starting with two key areas: Colorectal Cancer Screening and Breast Cancer Screening.
Introducing Your New Power Tools
CRISP DC is offering two main products to give you control over preventive service data for your patient panel: the Population Explorer and Bulk CSV Files.
1. Population Explorer: Daily Insights at Your Fingertips
Think of the Population Explorer as your dynamic, up-to-the-minute dashboard.
- Real-Time View: All users now have access to this new preventive services functionality, which shows you exactly which preventive services are applicable to the patients on your panel. The data updates daily, so you always have the freshest information available.
- Colorectal Cancer Screening: Data for this screening is available with no lookback restriction, meaning you can review historical information seamlessly.
- Breast Cancer Screening: Data for this screening is available for a lookback period since January 1, 2020.
This tool is perfect for quick checks and for identifying daily or weekly outreach priorities across your entire panel.
2. Bulk CSV Files: Data Delivered to Your Door
For those who need comprehensive data for large-scale campaigns or integration into other systems, we offer Bulk CSV Files delivered weekly via MFT (Managed File Transfer).
- Customizable Delivery: These files are currently delivered separately for each preventive service.
- How to Request Your File:
- Practices: Please contact your CRISP DC Account Manager to submit a request.
Understanding the Data Sources
The accuracy and utility of these tools are powered by diverse data flowing into the CRISP DC HIE. Here is a breakdown of where this critical preventive service data comes from:
- ADT Procedures: Procedures that have been performed at a reporting provider and sent to the HIE via an HL7 feed. Identification is based on matching codes.
- Diagnostic Reports: Procedures (often from transcription or radiology feeds) performed at a reporting provider and sent via an HL7 feed. These are currently identified based on the name description of the procedure. Crucially, details on the procedure, results, and even imaging may be available in the patient’s record.
- Structured Data / CCD (Continuity of Care Document): Data pulled directly from an organization’s Electronic Health Record (EHR). This data may identify procedures performed at the reporting organization, but it could also include patient-reported procedures from other care settings.
Important Data Limitations to Note:
To ensure you get the most out of these tools, please be aware of what is not currently included:
- The HIE does not include data from external sources that are not connected to the HIE.
- The system does not include data that is not coded with national standards (other than diagnostic reports).
- The current file does not include data from direct lab feeds or information on the result of the study (e.g., whether the colonoscopy was positive or negative). We are actively exploring ways to incorporate result data in the future!
Take the Next Step
These new Preventive Services products are a game-changer for improving care quality and patient outcomes. By leveraging the comprehensive, timely data from the HIE, your team can streamline outreach, ensure more patients receive essential screenings, and ultimately, close those vital gaps in care.
