Goodbye Structured Documents, Hello Visit Summaries!

You may have noticed the Structured Documents tab in the CRISP Portal….or maybe you didn’t. Honestly, you weren’t wrong to ignore it – it was poorly named.

Here’s the deal: that section was originally called Structured Documents because the data came through a CCD (Continuity of Care Document) feed. CCDs are encounter-level clinical documents such as visit summaries, discharge summaries, and progress notes sent by hospitals and providers (FQHC’s, specialists, etc.).

That name didn’t really describe what was inside, so, we’ve renamed it to Visit Summaries.

Under Visit Summaries, you’ll find:

  • CCDs sent directly from connected hospitals and facilities
  • CCDs received through national networks

It’s the same valuable patient data, now under a name that finally makes sense. We appreciate your patience as we work to make things more intuitive for you!

More Data, Better Care!

We’ve enhanced the Visit Summaries tab with expanded access to patient data. Now pulling from national networks and direct data-sharing partnerships, you’ll see even more valuable information from trusted key providers with continued improvements coming soon

What will I see?

Visit Summaries or Continuity of Care Documents from the following organizations are now accessible:
• MedStar Health
• Johns Hopkins Health System
• Children’s National Health System
• Kaiser Permanente
• St. Elizabeths Hospital Washington VA Medical Center
• Exact Sciences
