CRISP DC’s Health Related Social Needs solution allows users to seamlessly single sign on (SSO) to DC Health’s instance of Findhelp, LinkU—a comprehensive social care network platform that connects individuals to community resources and support services. CRISP DC also allows for screening assessment data and Z-codes to be easily shared from your organization’s native EMR to the HIE.
What is LinkU?
CRISP DC has partnered with the DC Department of Health and Findhelp to integrate LinkU, DC Health’s screening, referral, and resource directory platform, into the CRISP DC HIE. LinkU will be accessible via the CRISP DC Portal and InContext application and will allow users to:
- Conduct a social needs screening assessment
- Search for community resource information available in the District
- Send closed-loop referrals to community-based organizations
Screening and Referral data captured in LinkU will be shared back to the DC HIE for all care team members to view at the point of care in the social needs and care coordination subtabs.
Organizations that capture screening assessment data within their native EMR can share their screening assessment data with CRISP DC via a CSV file. Once ingested by the DC HIE we will display screening assessment data in the social needs data tab. If your organization is capturing Z-codes and is sharing CCDA’s with the DC HIE, CRISP DC will automatically parse out the Z-codes and present them at the point of care in our Social Needs Data tab.
If your organization has its own instance of Findhelp, you can easily share your Findhelp data with the DC HIE by adding an addendum to your network agreement.
